Tri-Town Community Action Agency is seeking an innovative full-time (40 hours/week) Medical Director and practicing Primary Care Physician (MD/DO) to act as a key leader and vital member of the Agency’s Core Leadership Team, and to guide the Health Center’s Clinical Team through a coordinated care transformation process to continuing as an exemplary Level 3 PCMH Practice. This position requires creativity and leadership by an adept manager who is well versed in EHR.  The position requires 50% time devoted to clinical hours (20/week) and 50% devoted to administrative hours (20/week)
The position of Medical Director Reports directly to the Tri-Town Community Action Agency’s Chief Executive Officer and is classified as an Exempt Employee. 
About Tri-Town Community Action Agency’s Health Center:  
The Tri-Town Community Action Agency is a private, non-profit corporation which has been providing a broad range of multi-purpose services and programs targeting individuals and families living at or below 200% of poverty since its inception in 1965.  The Mission of the Agency is “To provide community residents with accessible, quality, and comprehensive health, education, prevention, child care, nutrition, employment and training, economic security social and other support services in their self-directed goals toward transitioning out of poverty and into social and economic self-sufficiency.”   
Tri-Town’s Health Center provides Preventive and Primary Care Services, Mental Health and Substance Abuse Counseling and Treatment, Integrated Behavioral Health, Chronic Disease Risk Management and Prevention, Dental/Oral Health Services, Nurse Care Management/Care Coordination, Family Planning (Title X), WIC, Women’s Cancer Screening Program, WISEWOMAN, Pediatrics, Women’s Health, Ob/Gyn Services, Pre-Natal Care, Tobacco Treatment and Counseling Services, On-Site Laboratory Services, Medical Referral, Health Coaching, Patient Navigators, and Community Health Outreach to over 6,000 patients annually.                                                           . 
The Health Center is a designated Federally Qualified Health Center (FQHC); a Level Three NCQA Recognized Patient Centered Medical Home and is Licensed by the RI Department of Health and the RI Department of Behavioral Health, Developmental Disabilities and Hospitals. The Health Center is located at two different sites in the Towns of North Providence and Johnston. 

General Requirements and Qualifications:

General: The Medical Director is responsible for the supervision, leadership, and evaluation of the primary care provider team of physicians, mid-level practitioners, and other clinical staff as required and maintains a balance between administrative responsibilities and direct patient care. As an active member of the Senior Leadership Team, the Medical Director serves as a role model for  excellence and continuous quality improvement within the organization.  The Medical Director oversees and directs clinical quality of care, risk management, and regulatory compliance as per HRSA utilizing clinical best-practice guidelines.  Clinical and administrative experience working in an out-patient ambulatory health care setting with knowledge of Federally Qualified Health Centers is strongly preferred.

Specific Requirements and Qualifications: 
Must possess or be eligible to obtain a license to practice Medicine in the State of Rhode Island and have a valid DEA license. Must be Board Certified or Eligible and have a strong commitment to working with the medically underserved populations; 
Must be credentialed, or be eligible to be credentialed with insurance companies; 
Must have Acceptable National Practitioner Databank Results; 
Minimum of two (2) years of FQHC or similar experience;
Must have two (2) years of Management and Clinical experience;
Spanish-Speaking Bi-Lingual Candidate a plus;
High Proficiency in EMR required (NextGen a plus); 
Successful candidates will meet and possess the following:
An affinity for and knowledge of the Patient-Centered Medical Home Model of health care delivery and a desire to lead in the consideration and adoption of new, creative, innovative and cost-effective models of primary care;
Understanding and implementation of Evidence-Based Clinical Practice Guideline;
Ability to oversee Infection Control Plan and Patient Safety Plan;
Must be a TEAM Player and have a strong commitment to Patient-Choice;
Ability to demonstrate leadership among clinical staff in regards to quality clinical patient care, sound clinical documentation, patient satisfaction, and implementation, monitoring and reporting through EHR systems;
Skilled in oversight and review of the clinical practices of all staff physicians and mid-level practitioners through individual and group supervision, documentation and chart review, clinical care planning, continuity of care, patient engagement and provider relations, continuous quality improvement; ongoing peer review and training and technical assistance; 
Ensure the ongoing delivery of quality medical care and clinical services through utilization review and quality assurance;
Assess overall performance of the Health Center:  including clinical program review (UDS Data, CTC-RI initiatives) and maintain accreditation with State and Federal programs (PCMH/NCQA); 
Assist in maintaining, and expanding programs and funding applications to various state, local, federal and private funding; 
Ensure compliance with state, federal and local regulatory requirements as promulgated by HRSA, RI Department of Health, RI Department of BHDDH; Office of Pharmacy Affairs; Federal Torts Claims Act;  
Conduct ongoing support, mentorship, coaching, guidance, evaluation, leadership and peer review of medical providers;
Maintain Multi-Disciplinary Team approach to quality care;
Maintain and expand collaborations and linkages with major community stakeholders to enhance patient experience and maintain and expand specialty care and hospital referrals;
Provide quality prevention and primary care targeting the medically underserved living at or below 200% of poverty ensuring delivery of services with dignity, respect and compassion; 
Demonstrate knowledge, understanding and commitment to the Culturally and Linguistically Appropriate Standards (CLAS) of Care;
Demonstrate a passion and commitment to working with culturally, economically, politically and otherwise diverse patient population;
Serves as “Medical Champion” and leader of the Chronic Care Collaborative and the Integrated Behavioral Health Initiatives;
Serve as Physician Champion for systems associated with robust and integrated Electronic Health Records;
Establish policies, procedures and guidelines to monitor effectiveness of medical care, evidence-based clinical outcomes, and patient progress;  
Ensure annual review and update of clinical policies and procedures with approval by the Governing Board of Directors as required by HRSA; the Joint Commission and other accrediting and licensing bodies; 
Provide oversight, monitoring and feedback of the required Clinical Quality Measures, meeting threshold benchmarks as mandated by HRSA, HEDIS, NHPRI, Various Health Plans, Medicaid, Medicare and CTC-RI; 
Participate in all federal, state, local and other On-Site Audit Reviews;
Create (where needed), revise, update, implement and evaluate Clinical Practice Guidelines (based on best practice) as per the American Association of Family Medicine on an annual basis; 
Participate in CTC-RI Practice Transformation Committee Meetings in maintaining Level 3 status as a Patient-Centered Medical Home; Commitment to the PCMH Model of Care and working in an Integrated Primary Care setting with a Multi-Disciplinary Team of Providers;
Participate in the submission, on-going maintenance and evaluation of Patient Centered Medical Home and NCQA Recognition; 
Provide leadership and participate in monthly Meetings: Quality Improvement Committee; RIHCA Clinical Leadership Committee; PCMH Committee; Integrated Behavioral Health Committee; Clinical Care Team; Operations Committee; Health Center Staff and Leadership Committee Meetings;
Oversight of High-Risk/High-Utilizer Initiative; 
Assist in Continuous Quality Improvement through the Agency’s Quality Improvement/Quality Assurance Plan including planning, implementation and evaluation;
Participates in all Medical Provider Recruitment and Retention goals and activities; 
Assists in the submission of HRSA Five (5) Year Grant Application; HRSA Funding Opportunity Announcements; Annual FTCA Re-Deeming Application; 340B Pharmacy Program Site Submission, Re-Submission and Integrity Audits; 
Oversight of Programmatic, Clinical and Financial goals of the Health Center.


Additional Details:
Hours: FTE – 40 hours/week (20 hours of direct patient care and 20 hours of administrative); At least one evening per week and rotating Saturday shifts with other Providers required. 

On Call:  Light duty, shared among Providers and required to respond to 24 hour answering service in a timely and appropriate manner and to provide support to on call medical providers. Hospitalists are assigned to Tri-Town patients as per agreements with various state and local Hospitals. 

Salary/Benefits: Salary Range: $175,000 - $195,000 – commensurate with skills, knowledge, education and experience.  Tri-Town offers an excellent fringe benefit package which includes  sick, vacation, paid holidays, FTCA Malpractice Insurance, GAP Insurance; CME Allowance (financial and paid time off ), health/dental/vision benefits, life and long-term disability insurance, pension with employer match.
