Our goal is to open the doors and guide seniors towards the many programs that will improve their health, well-being, and quality of life.

This program helps seniors and disabled adults who are eligible for RI Medical Assistance and in need of Personal Care through the development, implementation and monitoring of their Individual Service and Spending Plan (ISSP). ISSPs promote independence by providing assessments of accessibility, equipment, and health education needs and assistance arranging necessary services.

Referrals are accepted on a statewide basis.
